Services

If you feel that your child may benefit from occupational therapy or speech-language therapy services, please contact our office at 405-840-1686 or via email at sensational.kids@hotmail.com. Our office staff will work with you on determining if therapy is an appropriate fit for your child.  A home packet will be provided for the parent to complete to further assess your child’s area of need.   After completing the initial packet, we will schedule your child for an evaluation.

Please call our office if you would like to set up an appointment to visit our facility.  Our staff will work with you on answering questions that you have regarding occupational therapy or speech language therapy at that time.

Before the initial evaluation:

  1. Complete and return the Developmental Intake Assessment.
  2. Complete and return the Sensory Profile
  3. Read, sign, and return the Notice of Privacy Practices Acknowledgement of Consent
  4. If you will be seeking reimbursement through your insurance company, please complete the insurance form provided in the home packet.  Please bring a prescription from your primary care physician and a copy of your insurance card for our records.

In addition to the information above, children covered through Medicaid will need the additional forms:

  1. A Medicaid Referral Form completed by the child’s primary care physician.  This form may be faxed directly to our office at 405-840-1006.
  2. A copy of the child’s Medicaid card.

Why is it important to fill out the lengthy Sensory Profile and Developmental Intake Assessment (DIA) and mail them back before the evaluation?

Since caregivers spend a great deal of time with their children, they know their child best.  Therefore, it is important for the OT/SLP to understand how children react to various sensory inputs in their daily lives, including home and school.  Although the OT/SLP will observe reactions to various sensory inputs during the evaluation, it is imperative to get a better picture of children from their caregivers.  Also, reviewing the information before the evaluation is helpful to the therapist when setting up the environment before the evaluation.  The therapist will discuss information reported in the Sensory Profile with caregivers during the evaluation.  Additionally, the DIA provides the therapist with the caregiver’s main concerns, which are used for planning treatment goals.

What happens after I return the paperwork?

Once the completed packet is returned to the office, your child’s evaluation will be set up.  However, if there are no appointment times available for regular treatment sessions, your child’s name will be added to a waiting list.  Since planning a child’s discharge from OT or SLP is difficult, it is hard to give an exact time frame for an anticipated wait.  However, most children are seen within a two to three-month wait.  Recent additions to personnel should greatly reduce the waiting period.  When an evaluation date is provided, it is important for you to verify OT/SLP service reimbursement with your insurance company.  Although OT’s can evaluate and treat children without a doctor’s prescription, many insurance companies later ask for a prescription.  Therefore, it is best to request a prescription for an OT evaluation and treatment.

What can I do for my child before the evaluation?

Having a well-rested child who is not hungry is usually best.  Make sure your child wears comfortable shoes; sneakers are best for running and balance activities.  Socks are required for interactions within the sensorimotor gym at all times.  If your child wears corrective lenses, make sure you bring them.  One caregiver is welcome to be present during standardized testing.  However, if you feel your child may do better if you are not present, there are two-way mirrors and a sound monitor available.  It is important not to break up standardized testing with questions or attempt to correct your child’s behaviors during testing (this is a hard one sometimes).  However, the therapist will provide paper and pen to make comments during the quiet times.  During play activities, the therapist will encourage caregivers to ask questions, make comments, or discuss concerns.

What will happen after the evaluation?

The therapist will discuss some general impressions and observations noted during the evaluation.  However, the therapist will need time to score the evaluation and Sensory Profile. Later, after the evaluation is scored, the therapist will discuss the results and recommendations.  A written report will be provided, with a copy sent to your physician if requested.  The therapist will also keep a copy, which will be sent to your insurance company, if needed.  If you choose to have occupational therapy or speech language therapy for your child, you will actively participate in weekly sessions, if you desire.  The therapist and caregivers will work as a team to assist each child in achieving his or her goals.

Payment Policy

Payment for the evaluation and treatment sessions are due at the end of each session.  Our office will provide an insurance form following each treatment session (or as requested by parent) for parents to submit to their insurance company to seek reimbursement for occupational therapy or speech language services.  Parents are responsible for filing all insurance claims.  Written reports will be provided upon request of either insurance company or parent.

Cancellation Policy

Since children and other family members understandably get sick, we will allow two cancellations per quarter (three month period).  However, if a family needs to cancel additional sessions during this treatment period, payment of a regular treatment session is required to keep their child on the regular schedule.
